Attention all families in grades 3-5 and Mrs. O’Connell’s and Mrs. Ryan’s classes:
In order for your child to receive their district issued iPad, it is extremely important that all iPad forms are turned in to your child’s teacher by AS SOON AS POSSIBLE.
The forms that need to be returned are in the iPad packet you received when attending one of our parent nights this week. The forms are colored golden for your ease and are:
1. Apple ID Form
2. Insurance Agreement
3. Student Acceptable Use Agreement
4. Participation Agreement
If you are selecting insurance, be sure to state you paid on-line or by attaching a check.  Be sure to clearly indicate whether or not you want your child to bring the iPad home.If one or more of these forms are missing or not filled out completely, it will delay the issuing of your child’s iPad.
