2022-2023 YALE Preschool School Year enrollment will be opening on June 1st at 8 am.
ENROLLMENT STEPS:
- Enrollment Forms- Please print YALE enrollment forms here. Complete, sign, and scan* YALE enrollment forms so they are ready to upload.
- *When scanning documents, please use a scanner or one of the many free scanning apps available through your smart phone. Please scan the YALE enrollment forms as one “batch”. Pdf attachments are preferred. Illegible documents will be rejected.
- Please note: Physician’s Report does not need to be uploaded. You have two weeks from the time your child starts to submit this form.
- All enrollments should be completed online at www.ezchildtrack.com/ostp/Parent using the desktop version of the web page.
